South Warwickshire and Worcestershire Mind is searching for a passionate and dedicated Safe Haven Practitioner to join our team. You’ll provide out-of-hours mental health support in a welcoming and calming environment, helping individuals navigate challenges and build resilience.
Through its twilight telephone and face to face support, the Safe Haven service aims to:
- Prevent escalation of mental health problems and thereby avoid mental health crisis
- Prevent unnecessary referrals to secondary mental health services, A&E departments and other emergency out of hours services
- Improve mental health and wellbeing
- Increase independence and self- management
- Reduce isolation
Main Responsibilities
- To provide 1:1 confidential, emotional support as required both over the telephone and face-to-face
- To provide advice and emotional support to individuals
- To provide a friendly welcome to the Safe Haven to new referrals
- To carry out initial assessments and maintain accurate case records
- Help individuals develop self-management strategies
- To signpost and refer to alternative support as required
- Provide engaging and relevant activities on site for the individuals attending
